fix: replace #123 with #<issue-id> in PR template comments to prevent auto-linking The PR template comments contained 'Fixes #123' as an example, which GitCode/AtomGit parses as a real issue reference even inside HTML comments. This causes every PR to auto-link to issue #123, and once that issue accumulates 100+ PR references, all subsequent PR creation fails with 'An issue cannot be linked to more than 100 PRs'.
