| fix(ui): refine chat composer layout, file chips, and session list
- Composer: stacked layout (textarea top, attach/send toolbar bottom), 80px
min height, tighter padding, white/bg-primary input zone, no dividers.
- File pills: integrated progress fill, content-width with 160px max, 14px
body-style filename, formatFileSize helper.
- Toolbar buttons 28×28 with 16px icons; send uses btn-primary.
- Remove green status dot from selected session in Agent chat sidebar.
